Patient Information
Appointments It is requested that you arrive a few minutes early for your first appointment so that the necessary registration and insurance information can be obtained. Please bring all insurance information with you so that we can process our forms in a timely manner. Each surgeon makes an effort to see patients at the scheduled appointment time. There are occasions when a patient needs more time than anticipated, or an emergency requires our attention at the hospital and as a result you may find yourself waiting. We assure you that every attempt is made to respect your needs. If an appointment needs to be changed, we ask that you call and reschedule at least 24 hours in advance. If you need to be seen quickly because of a medical emergency, we will see you on a same-day basis in the office or in the emergency department.

We are happy to process all insurance forms for you as long as you have provided us with the appropriate insurance information. We expect payment promptly if you receive a bill from our office for balances that may be due. Please feel free to discuss any questions you may have about billing with the receptionist or the billing manager. Dr. Bernstein and Dr. Lanchoney are providers for most major insurance plans. However, it is in your best interest to check your insurance providers list of participating physicians to be sure your plan allows you to be seen at our office. Also, please review with your insurance customer service representative (or insurance provider) the rules and limitations on coverage and reimbursement.
